The Annual Subscription for each team entered is as follows:

Submit your paperwork & payment prior to August 31st, and the fees outlined above will include registration fees of the players being registered at that time. After August 31st player registration fees are £5.00 for 11 A Side and £3.00 for competitive Mini Soccer.

> an invoice for the payment required to join your clubs and your teams to NEHYL

> statements for you and your Club Chairman to sign and return

> copies of the League’s Privacy Policy, and Child Protection Statement

NEHYL Privacy Policy - Please read this policy carefully to see how we will treat the information that you provide to us. We the North East Hampshire Youth League (the NEHYL) will take reasonable care to keep your personal information secure and prevent any unauthorised access or unlawful use of it. We process all personal information in accordance with applicable UK data protection legislation.

We will use your personal information to register you as a player/manager/official (as appropriate) in the NEHYL and will hold your personal information (your contact details, address, telephone number(s) and e-mail address) on the NEHYL database and in the NEHYL Directory. If you do not want your e-mail address to be published on the NEHYL's website, please tick the opt out box on the Application for Membership Form/Team Form (as appropriate).

We may provide your personal information to other organisations who may send you information on football related matters that may be of interest. If you do not wish us to pass on your information, please tick the opt out box on the Application for Membership Form/Team Form (as appropriate).

If any of your information changes you must update the NEHYL so that an accurate record can be kept.

SPECIAL NOTE

From Season 2008/9 onwards a Club’s entry into NEHYL can only be effected by use of this form, handwritten forms are no longer acceptable.

Most fields in the form are MANDATORY and leaving fields blank or the use of dashes and zeros is liable to lead to your application being rejected - remember “if we didn’t need the information we wouldn’t be asking the question”
